STUDENT REQUIREMENT FORMS
- Student HandBook: The student Handbook must be signed by a parent/guardian and student and turned into the main office.
Medical
- Physical Health Examination: An up-to-date physical exam should be obtained from your child’s physician (Please note, 9th Graders also need to submit a record of dental exam by the end of the semester)
- Vaccination Records: An up-to-date immunization record should be obtained from your child’s physician.
